As a new patient at Waushara Dental Associates, you can access the following items on our website.
During your initial visit, we will reserve one hour to take the time to learn about your individual needs and concerns. Prior to your visit, please print and complete the new patient forms. If that is not possible, we also have copies at our office. Please arrive 15-20 minutes early to allow enough time to complete your paperwork. Please bring along a list of your medications and alert our office if you have a medical condition(artificial heart valve, joint replacement, history of infective endocarditis, heart transplant, certain congenital heart conditions) that may require antibiotic pre-medication prior to your visit. During your comprehensive exam, a complete set of digital x-rays will be taken so we have a comprehensive view of your teeth and supporting structures for accurate diagnoses. Before you leave, we will present you with a treatment plan, explain your options for care, answer all of your questions, and schedule treatment. Please call Waushara Dental Associates to reserve your preferred date and time. We'll call to confirm your appointment two days in advance. We kindly ask for at least 24 hours notice to reschedule appointments. If you must reschedule due to an emergency, please call us as soon as possible so that we may offer your appointment to another patient.
Waushara Dental Associates, strives to help you get the smile you've always wanted. We offer a number of payment options. For your convenience we accept all major credit cards and debit cards. Third party financing is available through CareCredit. We do require payment at the time of treatment. We offer a 3% discount for payment made in full by cash or check at time of service.
As a courtesy, we'll gladly submit your insurance claim. You are responsible for your deductible or co-pay, as well as any unpaid balance not covered by insurance. We are a participating provider of Delta Dental (Premier), WEA, and Humana Dental.
